We respect the right to privacy of all visitors. By visiting the Web site of the Second Wind Lung Transplant Association of St. Louis (the Association) at www.secondwindstl.org, you are accepting the practices described in this Privacy Notice. This privacy statement applies only to the Association Web site and we are not responsible for the privacy practices of other Web sites you may link to through our site.

The Association respects the privacy of every individual who visits our Web site. This privacy policy outlines the information we may collect and how we may use that information.

Personal Data

The Association will not collect any personally identifiable information about you (e.g., your name, address, telephone number, or e-mail address) through our Web site unless you have provided it to us voluntarily. If you do not want us to collect your personal data do not provide it to us.

Personally Provided Information

If you choose to provide us with personal information by sending an email, or by filling out a form with your personal information and submitting it through our Web site, we use that information to respond to your message and to help us provide you with information or material that you request. On occasion, we may conduct a study concerning the types of questions sent to us. These studies help us to improve our Web sites in order to make them more responsive to the needs of our users. We do not give, share, sell, or transfer any personal information to a third party.

Aggregate information gathered from your visit is used to better design our Web site. Donation Information

The Association maintains records of all contributions. While personal information is private and confidential, the names of all contributors are listed on the Friends of Second Wind page of our Web site. Please notify us in writing if you do not wish to have your name listed on this page.

Newsletter

We encourage you to subscribe to the Association’s Newsletter. An email version and a US Post Office mail version of our newsletter are available. If you wish to subscribe to the Association’s Newsletter, we require contact information such as name, address, telephone number and email address (when appropriate). The email version of our newsletter is free. For those who do not have email, and would like a newsletter sent to them, a membership donation is required to cover the cost of mailing.

Web Site

The Board of Directors for the Association is ultimately responsible for the content of the Association’s Web site. A standing committee comprised of board members and members of the organization work collaboratively to update and modify the Web site using a systematic process.
The Association’s members, board members and the lung transplant medical staff at Barnes-Jewish and St. Louis Children’s hospitals are consulted as to specific topics of medical articles for the Web site. Medical articles are reviewed annually for appropriateness of topic and information. This process is conducted by physician peers, lung transplant nurses, and coordinators who are certified nurse practitioners.
Sources of funding do not have any influence on the editorial content of the Web site. There exists no conflicts of interest as the process is completely transparent.

Third Parties

We do not share any personally identifiable information we receive from you with third parties unless you instruct us to do so.

Endorsements

The Association does not endorse any specific commercial product that may be mentioned on this Web site.

The Association does not use third party advertising services.

Sponsorship and Donations

Access to the Association’s Web site is provided as a free service to those seeking information relating to lung transplantation. Sponsorships and donations are necessary to continue to achieve the goals of the Association. The Association receives its primary funding from the sale of Donor Awareness T-shirts, Annual Second Wind Lung Walk, Membership Donations, Individual Donations, and Corporate Sponsorships.
Please consider a sponsorship or donation to the Association. Funds can be given as a one-time gift or regularly scheduled contributions. The Association is a 501(c)(3) nonprofit organization and all donations are tax-deductible. The Association reserves the right to change this policy at any time. Our privacy policy will be kept up-to-date and clearly posted on our Web site.
